Invisalign and other orthodontic treatments are life-changing — I’ve watched patients light up when their new smile comes together. But there’s a conversation I sometimes have to have first, and it catches people off guard:
“Your oral hygiene isn’t quite ready for this yet.”
I know that’s not what anyone wants to hear when they’ve set their heart (and budget) on straighter teeth. So let me explain why we say it, and why it’s one of the most patient-first things we do.
At our practice, no one starts orthodontic treatment until we, as hygienists and clinicians, are confident their gum health can support it. That’s not gatekeeping — it’s protecting your result.
Think of it like building on solid ground. Orthodontics moves your teeth through the bone that holds them. If there’s active gum disease underneath, you’re moving teeth through compromised foundations, plaque gets trapped around aligners and attachments, and the risk of the whole treatment failing goes up dramatically. You’d be paying thousands of pounds for a result that may not last.
If we think you can keep everything clean — and the treatment will succeed because of it — brilliant, off you go. If not, we’ll be honest with you.
Because we accept direct access hygiene patients — people who aren’t registered with our practice but come in for a hygiene appointment on a pay-as-you-go basis — I regularly meet people who started Invisalign elsewhere.
Too often, I look in their mouth and my heart sinks. Their oral hygiene simply isn’t at a level where that treatment can succeed. And I have to have the honest conversation: “If you’d been our patient, we wouldn’t have let you start this yet — because you’re not going to get the best out of what you’ve paid for.”
I’ll be blunt: I’ve heard of practices that will just take the money, and the result will be what it’ll be. That is not how we work, and I don’t think it’s how anyone should work.
Here’s the thing patients sometimes miss in the moment: a “not yet” is not a rejection. It’s a plan.
We’ll show you exactly where the problems are — mirror in your hand, so you can see it yourself — get your hygiene routine working, bring your gum health up to standard, and then give you the green light. Sometimes it takes a step backwards to take two forwards. The result is Invisalign that actually works, on a mouth that can keep it working.
And something lovely happens along the way: patients tell me no one had ever been straight with them before. Many of the direct access patients I’ve had that conversation with come back to see me again — because honesty builds trust. Sometimes you need to not sugarcoat it (pun very much intended).
Start with a hygiene appointment. Best case, we confirm you’re ready and you begin with the odds stacked in your favour. Otherwise, you’ll leave with a clear, achievable plan to get there — and a team that will tell you the truth at every step.
